Admitting and registration are conveniently located just inside the Emergency/Outpatient entrance to the hospital, through the doors under the blue awning. Registration personnel are available from 6:15 a.m. until 10:30 p.m. to assist patients with the registration process. Plan to arrive 15-20 minutes early to insure timely processing.

During your registration for medical services, information and authorization will be needed for your medical record and for financial arrangements. All information will be handled confidentially. Please bring any insurance materials including your insurance and/or Medicare cards and any co-pays or deductibles, if owed.

If you are coming to the hospital for tests, you should also bring the physician's orders for the test(s). Children who are brought to the hospital by someone other than a parent or legal guardian must bring written consent for medical treatment from a parent or legal guardian.

Other helpful information to bring with you includes a medical history (previous surgeries or medical conditions, allergies, etc.), a list of medications you are taking and a copy of your advance directive if you have one. See Advance Directives for more information.
